It will probably mess up your night vision. If you have to fire out the passenger window of a car, it will probably harm the passenger. I would be very careful about shooting close to the body like if somebody had grabbed you, you may burn your body or possibly your face.
Basically, anything above the barrel in an arc risks getting harmed.
I don't know how bad it would be. You may want to test a ported barrel before you get yours done to see how bad the damage could be.
I'm not a big fan of ported guns in general, but I would think that a Kel-Tec is not really constructed sturdily enough to be modified in this manner. I'm not saying they're junk (I have a P3AT myself), but I certainly don't see it as a cost-effective (or tactically effective, for that matter) modification.
