Mailed: 17 January (not certified mail, I don't know when DOA received it)
Check Cashed: 18 February
Issued: 6 April
Rec'd: 17 April

90 days on the nose, mailbox to mailbox.

I'll add to that +infiniti on calling and speaking to a supervisor. After experiencing this process, I'm convinced I'd still be waiting for a issuance if I hadn't called on 6 April.
